Please bring the following for your Immigration Physicals:

  1. Form I-693 (attached below). Complete the first page and the top line of every page thereafter in black ink.

  2. Completed Demographics sheet. (attached below)

  3. Completed HIPAA form. (attached below)

  4. Vaccination records if available including COVID card, Tetanus, MMR, Varivax and Hepatitis B vaccine records.

  5. Government issued photo ID.

All attachments below must be filled out and brought to your appointment
